In accordance with Texas House Bill 4224 and Texas Health & Safety Code Chapter 181, Spencer Counseling Services provides consumers with the following information regarding access to health records and consumer protections.
Requesting Your Health Records
Clients may request access to their health care records by submitting a written request to Spencer Counseling Services.
Requests can be submitted by:

- Include your full name, date of birth, and the specific records you are requesting.
- Requests may be subject to identity verification and applicable fees as permitted by law.
- Records will be provided within the timeframe required by Texas law.
- Requests will be processed in accordance with applicable state and federal laws.
If records are requested on behalf of another individual, documentation verifying legal authority is required. Examples include guardianship documentation or medical power of attorney.

Filing a Consumer Complaint
